To cut Medicaid? – New York Times

Republican leaders in the Congress directed the committee that controls Medicaid to reduce $ 880 billion from the next budget. They say that these deductions are not necessarily targeted 72 million poor and disabled American insurance program in Medicaid. For example, interruptions may come from Medicare. However, Trump promised not to touch this very popular program. And this can not come from anywhere else.
The Republican process is just beginning and we do not know how the deputies will change the program. Most Medicaid money goes to the states, so the best way to consider the proposal is a deduction on state budgets. State deputies can react by leaving the scope, increasing taxes or cutting other parts of their budget. I will explain a few possible scenarios in today’s newsletter.
Who is closed
Medicaid is designed to divide the medical invoices of a patient: Each of the Federal Government and the state will pay a certain share. (The contribution of a state depends on how poor it is.)
The law is certain about what Medicaid has to include – for example, cancer scans and kidney transplants, but not prosthetic legs – and republicans cannot change it with a budget bill. Each state must cover certain population, including patients, pregnant women, disabled people and patients without money.
Most states also choose to cover an optional group added as part of Obamacare in 2014: Anyone who earns less than a particular income (about $ 21,000 for a single person). Republicans want to apply a study to this group for non -disabled people. This idea is popular among the people, but the federal government is not enough to achieve the GOP target.
LARGE TARGETS
Something more to reduce the share of the federal government will bring burden to states. And MPs may take care of their own ways. They can cut optional populations such as Obamacare group. In case of federal financing in twelve states, it has laws to do so automatically. If they do not want to leave people, states may leave optional benefits such as prescription drug scope.
After these deductions, states face difficult elections.
They can pay less for doctors, hospitals and care homes for care. But there is a limit. If Mississippi suddenly starts to pay $ 50 for an echocardiogram instead of $ 160, cardiologists may stop seeing medicaid patients. (Many Medicaid patients are already struggling to find care because the program pays very little money to doctors.) Such interruptions can dismiss some nursing homes or rural hospitals.
Nevertheless, states would still need much more money for Medicaid, often the biggest expenses after education.
Where can they buy? They would have to sacrifice other priorities. An option is to cut training. Another is to increase taxes. None of them will be necessary by the Federal Legislation; It is up to the states of how they deal with. This enables the Republicans in Congress to say that they do not reduce medicaid benefits or fitness, even if most places are inevitable.
Very big to fail
Republicans say that the original agreement between Washington and the states is worn out and covers more than the shares of the FEDs. This is true. From beginning to end Various accounting Gimmicks, states have reduced the Medicaid contributions and now pay about one -third of the bill on average. In addition, the Washington 2014 Obamacare expansion has undertaken almost all costs.
However, this expansion made Medicaid popular. More than half of the Americans say that someone in their families uses the program and only 17 percent support to reduce the budget. Local MPs will probably not win voters by chopping or collecting taxes to save Medicaid. Therefore, the Democrats settled in Medicaid as the best speaking point about the GOP budget plan.
In 2017, Republicans tried to cut Medicaid’s budget. The base opposition helped to overcome the effort like a comprehensive lobby made by the governors of the Republic, who called on the senators not to leave them with a large financial hole.
The popularity of this bill – and its failure – helped the democrats back the house next year.
